Do you or your teen have concerns or questions about the transition from child to adult health care?  If, so this conference is for you!   Join us for a full day, FREE conference  for parents and youth! Teens with special needs ages 12-17 are invited to attend with a parent.  Special sessions, activities, and prizes for teens!   These day-long parent and youth conferences will offer information, ready-to-use planning tools, and resources for youth, families, and providers to make the health care transition process easier and less overwhelming.      The conference will be offered in 3 different locations.
